REGISTER YOUR TRIP
As soon as you have received an official invitation from an Assemblies of God World missionary, please register your trip by sending the MAPS Trip Registration form to AGWM. This will allow time for the Team Application Specialist to communicate with your AG missionary for approval. If you are working with an AG missionary, you are REQUIRED to register with AGWM and purchase GTL insurance. Please see information regarding the required GTL insurance below. Only general information is needed at this time.
Your Team Application Specialist will email you a confirmation number after she receives your Trip Registration Form.
The form may be filled out on-line and returned by one of the following methods:

- Mail to AGWM-PMC, 1445 N. Boonville Ave., Springfield, MO 65802
TEAM PROCESSING
IMPORTANT: All team forms need to be received by AGWM at least one month prior to departure. Adding team members within two weeks of departure may result in delay of the application process.Once your trip has been registered and you have received your confirmation number follow the steps below to process your team:

Step 2
| Disperse the Registration Packets to all team members. (Click on the links above to download.)

Step 3
| Screen all adult team members through www.NSOPW.gov or with a criminal background check.

Step 4
| Obtain Team Member Signature Form for each member.
Note: Team members with a volunteer card do not need to complete the form.
Only a copy of their card will be needed, unless they would like to update their contact information.

| | Obtain signed and notarized Minor Registration forms for each team member under the age of 18.
Note: Temporary Guardianship form is only needed for minors not traveling with a parent/legal guardian. |

| Step 5 | Mail only the following items to AGWM: |
| | - Insurance List Report (one per team)
- Notarized copy of the Team Processing Form
- Copy of each team member's Team Member Signature Form and/or copy of volunteer card

| Step 6 | Keep all original forms on file for at least five (5) years. |
INSURANCE PAYMENTTo make an insurance payment ($3.00/person/day, including days of travel), please call the AGWM Insurance Specialist at 417-862-2781 Ext. 3136. You will need to have the following information ready:
- Trip information (confirmation number, dates of travel, number of team members, destination).
- Credit card information (type of card, address associated with card, credit card number, card expiration date).
- Email address where confirmation may be sent.
You may also send a check along with your forms. Make the check payable to AGWM and put Account # 302959-2 (08) in the memo line.

GTL insurance is required as part of the registration process. GTL insurance coverage is not intended to replace a team member's current coverage but is in addition to their coverage. In case of a medical emergency or accident, the host missionary takes care of making the arrangements for medical treatment and/or evacuation with AGWM's assistance. AGWM has a long relationship with GTL due to their high standards and low cost. Therefore, we require all teams to be insured through GTL.
CONSTRUCTION TRIPS
World Missions requests that all MAPS construction project funds be sent through the MAPS Construction office. Please download the MAPS Construction Projects form and include with your check made payable to MAPS Construction. WORLD MISSIONS GIVING CREDIT
Processing a team prior to departure ensures the opportunity for AG World Missions Giving Credit and insurance for team members.
If your church would like to be considered for World Missions Giving Credit, send your team Expense Report to Chris Prost within one month after returning to the states.
